Access to the lock internally is very difficult. In my case the rod from the external door handle to the lock had been broken off. Removal was very difficult, but I found that if you had the window up, the rear window rail can be slid upwards if you remove all of its mounting screws. gently pull the top of the rail towards the front of the car, and at the same time push this rail upwards from the bottom. It is tight, but when pulled up the lock is so much easier to remove and install. A simple mig weld was all that was needed to fix this problem. Also take great care with the central locking switch mounted on the key cylinder, inside the door. Mine was brittle plastic and guess what, it broke (how did you guess that). New switch assembly around $100 but nil stock at my toyota dealers database. Found friendly wrecker and ordered used lock and switch for $80.
