Dad had this happen once too on one of his old Triumphs.
Basically second gear was just plain old worn out - worn bushes, worn synchro, worn gearing. Had to have the whole box rebuilt as some of the other gears were on the way out too, cost a fortune way back then, and that was with him pulling the box and putting it back in himself.
Assuming its the same thing, might be better to locate a good second hand box?
Maybe first of all check the linkages to make sure its not something simple like that though?

There are only two shifter bushes in the whole system - one for the shifter and one for the linkage in the top plate. IF these are worn (highly unlikely) then you need to remove the box to replace.
Alternatively, the springs that provide tension on the shifter rods to hold the box in gear may be broken, or not providing enough pressure on the balls to keep the shifter rods in their position. The shifter forks could also be worn out and not engaging properly, or chipped off on the synchro hub mating surfaces causing a lack of engagement.
You will need to remove the gearbox and take it apart for an inspection. Remove and Refit gearbox is around $300-$400 and it will take about 1hr to remove/refit the casings on the gearbox to get to the problem + more cash to fix the problem.
Most parts for the R154 are available from toyota... you would be surprised how much stock they hold for jap imports!
